We are excited to offer your age‐under‐18 sports team the opportunity to be fitted for customized protective mouth guards in our office!  

The prize:

‐ Mouth guards for all eligible team‐members and up to two coaches

‐ A mouth guard case for each mouth guard provided

‐ A team oral injury emergency kit

‐ Oral protection education and information for team members, coaches and parents
